Out and About with a Little Help from South Wilts Mencap

South Wilts Mencap (SWM) want to help make a difference by making it that little bit easier to get out and about with your family member or resident. South Wilts Mencap will contribute towards the cost of your outing with one or two people who have profound and multiple disabilities (PMLD) aged 18 and over.
South Wilts Mencap will fund outings on the following basis:-
- The individual has profound and multiple disabilities, is aged 18 and over and lives in Salisbury or South Wilts.
- Carer’s time up to 4 hours including travel time.
- A maximum of two carers for each individual.
- Transport to and from the individual’s place of residence. If own transport is used, we will calculate the return mileage from the individual’s home to the outing destination and reimburse at £0.45 per mile.
- A maximum of one outing per month, for any one individual.
- Receipts for transport and entrance fees will be required before expenses are reimbursed.
- The cost of refreshments will not be included.
Once you have decided on your destination and date, simply complete the form and submit it to them by email or can you pop it in the post.
