As part of Wiltshire Council's review of services for the Disabled Children & Adults Pathfinder, this pilot version of the single assessment and plan was presented at the WPCC/Wiltshire Council Pathfinder Event on 19th November 2012.

Although this is a working document only and NOT the final assessment, it shows how the plan focuses on the child/young person’s whole life rather than looking at one specific area.
